Named Accountable GP for All Patients

Every patient has been allocated a named GP who is responsible for your overall care at the practice. If you would like to know who your named GP is, please contact reception and they will be happy to inform you.

If you wish to change the name of your accountable GP, please let us know and we will make every  reasonable effort to accommodate this request.

Having a named accountable GP does not restrict your entitlement to see any doctor in the practice.

New Patient Registration

We accept new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.  

Please allow our staff 24 hours to process your registration before requesting an appointment.

Medical treatment is available from the date of registration.  Please contact reception for further information.

New Patients must complete the following two brief forms. Please note there are separate additional information forms. One for all patients aged 13 and over and one for children aged 12 and younger:

And

Registration - Additional Information

Or

Registration - Additional Information - Child 12 years old and younger

Once complete, please bring these two forms to the surgery reception.

We also request that new patients bring some identification which will allow us to accurately record your details on our system.  A photo-card drivers licence would be great as it shows both your name and address.  We understand that this is not possible for everyone and we will register you even if you are unable to provide identification.
