Named Accountable GP for All Patients

Every patient has been allocated a named GP who is responsible for your overall care at the practice. If you would like to know who your named GP is, please contact reception and they will be happy to inform you.

If you wish to change the name of your accountable GP, please let us know and we will make every  reasonable effort to accommodate this request.

Having a named accountable GP does not restrict your entitlement to see any doctor in the practice.

Whichever system your choose, repeat prescription requests will be made ready for collection after 3 full working days. You may deposit your Repeat Prescription Counterfoils at the surgery, indicating the item(s) required, or send your requests by post, or use either of the online links above.

The Dispensary at Roche is open from 08.30 - 12.45 and 13.30 - 18:00.  We can also arrange for prescriptions to be forwarded to local chemists for collection if you have difficulty attending.

Please Note: Telephone requests may lead to errors and are therefore not normally accepted.

Please click here for a form for those patients who will experience difficulties collecting their prescription anywhere other than a dispensary at one of our sites. This form will have to be completed by the patient and then handed to a dispenser in order for us to complete the final page (Part B).
