Named Accountable GP for All Patients

Every patient has been allocated a named GP who is responsible for your overall care at the practice. If you would like to know who your named GP is, please contact reception and they will be happy to inform you.

If you wish to change the name of your accountable GP, please let us know and we will make every  reasonable effort to accommodate this request.

Having a named accountable GP does not restrict your entitlement to see any doctor in the practice.

We would like to suggest that all of our patients start to use the NHS App to order medication.

You do not need a smartphone to use the NHS App, you can also access the same services in a web browser by logging in through the NHS website.

You must be aged 13 or over to use the NHS App.

Whichever system your choose, repeat prescription requests will be made ready for collection after 3 full working days. You may deposit your Repeat Prescription Counterfoils at the surgery, indicating the item(s) required, or send your requests by post, or use either of the online links above.

The Dispensary at Roche is open from 08.30-12.45 and 13.30-18:00. If you are a dispensing patient then you can also request to collect your prescriptions from our Bugle or St Dennis branches when they are open.

Please Note: Telephone requests may lead to errors and are therefore not accepted.

This form will have to be completed by the patient and then handed to a dispenser in order for us to complete the final page (Part B).

Bugle dispensing patients

As many patients have now been made aware, from the 16th February 2024 we will no longer be able to dispense to anyone within a 1.6km/1 mile radius of the new Banns Pharmacy at Rockhill, Bugle. The patients that this affects should have been sent a letter from NHS England/Cornwall & IoS ICB.

Please note that you are free to use any community pharmacy of your choice.

If you think that you will experience difficulties collecting your prescription anywhere other than a dispensary at one of our sites, Part A of this form will have to be completed by you and then handed to a dispenser in order for us to complete the final page (Part B). It will then be up to the South West Pharmacy Team at NHS England to make the final decision.

Serious Difficulty Application form

Please note that the decision to licence a pharmacy in Bugle was not made with our co operation and is out of our control unfortunately. We did appeal the opening at each stage, but the decision to go ahead was made by the wider NHS. We understand that lots of our patients would rather collect from us, but our hands are tied and we are not legally allowed to dispense to anyone who falls in the catchment above.

For any patients who don't live within 1.6km/1 mile of a pharmacy, we can continue to dispense to you at any of our branches.
